Annual Wellness Exam

Why is it important?

It is a federally mandated service which provides preventive and comprehensive health services for children from birth up to age 21 who are eligible for Medicaid. It provides critical services to improve the health of infants, children and adolescents. It provides a way for children to get medical exams, checkups, follow-up treatment, and special care they need to make sure they enjoy the benefits of good health.

When do I take my child?

• 3-5 Days after Birth
• Birth to 1 month
• 2 months
• 4 months
• 6 months
• 9 months
• 12 months
• 15 months
• 18 months
• 24 months
• 30 months
• Once yearly ages 3-21 years of age
